Thank you for considering us to be your family doctor.

We recommend enrolment with us for continuity of care and subsidised consultations. After enrolment forms are completed, we will arrange transfer of your medical notes from your previous GP.

There is no need to come in or book an appointment to enrol with us.

How do I enrol?

There are 2 Options:

Option 1 – Complete and submit the online enrolment form here.

Option 2 – Bring in the completed paper copy of the Enrolment Form from home (or fill it in at Greenwood Medical Centre), along with a copy of your proof of eligibility (e.g. passport)

• Parents can enrol and sign for their children under 16 years of age. Children 16 years or over must sign their own form.

The eligibility criteria are listed on both enrolment forms. Please tick the one that applies to you. For further information on eligibility, please refer to the Procare Enrolment Guidelines or Ministry of Health Eligibility page.

Casual Patients

If you do not meet the enrolment criteria and wish to be a casual patient and attend this practice, please advise our receptionist. Casual consultation fees will apply.
